When Agent Smith is interrogating Neo, after Smith has sealed Neo's mouth shut and he is backed into the corner, when the camera cuts back to Smith you can clearly see a reflection in his glasses of Neo still sitting down in the chair.

Revealing Mistakes
In the jump scene when Neo is running at full speed, you can see the CGI cutout of him from the background. It looks almost like a net around him.

Continuity
After Neo is shot by Agent Smith, Trinity leans close to whisper. Her hair begins to fall over her face. Cut to another angle and her hair is back in place again.

Revealing Mistakes
During the fight scene in the subway, as the Agent is about to punch through the concrete post, the section to be punched out is clearly visible.

Factual Mistake
During the flashback scene where Morpheus is describing to Neo the "peak of civilization", cars can be seen driving on the left hand side of the road. This is due to the filming location being Sydney, Australia; and in fact that footage is of the Sydney Harbor Bridge.

Miscellaneous
The doorknob at the Oracle's apartment is installed backwards. The screws are on the outside of the door, allowing a passer-by to take off the plate and remove the doorknob. Normally the screws would be on the inside to prevent this.
Trivia:
According to The Wachowskis, all animals in the Matrix universe are computer generated images.

The spring-loaded cell phones used in the film were Nokia Stilleto's or 8110's. These phones were produced in limited quantities and were only available in Europe and in Australia.

While asleep (inside the Matrix) at the beginning of the movie - one of Neo's illicit computer searches revealed that Morpheus had eluded authorities at Heathrow airport.
